Best Start Simcoe County GIS Initiative

  • Best Start is an integrated system of services that support families with children from birth through the transition into school.
  • Community Connection’s Resource Specialists worked with the County of Simcoe on a GIS mapping project of the services for children 0-6 years in Simcoe County and provide continually updated community resources through a software tool called an API. https://maps.simcoe.ca/Public/?MODE=theme&THEME=BESTSTART.

Child and Youth Crisis Line - Nipissing, Parry Sound & Muskoka

  • Hands TheFamilyHelpNetwork.ca and its Core Service Delivery Partners and Community Mental Health partners are continuing to work toward the transformation of Child and Youth Mental Health (CYMH) in the Muskoka, Nipissing and Parry Sound service area.
  • The group have launched a 24/7 Child & Youth Crisis Line for the regions of Nipissing, Parry Sound and Muskoka.
  • 211 provides the answering service for the line, conducting an assessment of callers and following protocols to connect callers to the nearest available assistance
  • 211 Central East, 211 North and 211 Central are the service delivery partners for this region.

Common Point for Mental Health & Addictions - North Simcoe Muskoka LHIN

  • 211 is the Common Point to access information and referrals to mental health and addictions services for children, youth and adults in North Simcoe Muskoka
  • 211’s Community Navigators provide bilingual service, 24/7, with multilingual interpretation in more than 150 languages.
  • Established by the LHIN’s Mental Health and Addictions Coordinating Council to create a single point of telephone contact for information and referral for all NSM LHIN mental health and addictions services for all ages.
  • Community Connection’s Community Navigators:
    • Follow established protocols tailored to local agencies, crisis lines, 911, etc.
    • Provide mediated assessments to ensure callers are connected to appropriate resources and referrals to other supports such as food, social supports, transportation, and financial assistance.
    • Offer advocacy and/or follow up calls to ensure callers make connections to services; including warm transfer options to ensure at risk callers are connected.

Common Point Mental Health and Addictions Online Directories – North Simcoe Muskoka LHIN

  • Custom online bilingual directories of mental health and addictions services for children, youth and adults complement the Common Point telephone access.
  • Community Connection’s Resource Specialists designed, developed and maintain the community resources which are accessed through a Common Point logo and online link placed on service provider’s websites https://mentalhealthandaddictions.cioc.ca/.

Grey Bruce Children’s Alliance Online Mini Directories

  • The Alliance is a network of organizations that work together to offer a more seamless, integrated system of services and supports to children, youth and families in Grey Bruce.
  • Community Connection’s Resource Specialists developed and maintain the online content for mini directories of resources for children, youth and families http://greybrucechildrensalliance.com/services-resources.cfm.

Grey Bruce We C.A.R.E Directory Project

  • An initiative of community agencies and school boards working together to increase community capacity and mobilization around the promotion of youth mental health and prevention of youth suicide.
  • Community Connection’s Resource Specialists developed and maintain the online content for three mini directories of resources for youth http://www.wecaregreybruce.ca/realindex.php.

Local Immigration Partnership – County of Simcoe

  • The County of Simcoe’s Local Immigration Partnership supports the development of local partnerships and community-based planning around the needs of newcomers. The Partnership works to improve access to, and co-ordinate effective services that facilitate immigration settlement and integration, improve access to labour markets for immigrants, and strengthen local capacity to integrate immigrants.
  • Community Connection’s Resource Specialist’s participated in the design and development of the immigration portal and maintains the community resources accessed through the site http://immigration.simcoe.ca/.

North Simcoe Muskoka Home and Community Care (nsmhealthline.ca)

  • Since the 1996 inception of the CCAC Simcoe County, Community Connection has been contracted for access to a county-wide community resource database.
  • This relationship evolved into merging community resource databases when the Simcoe and Muskoka CCACs were amalgamated around 2006, and included the development of a CCAC website named northsimcoemuskoka.info.
  • In 2013 Community Connection supported CCAC’s database and website transition to the bilingual nsmhealthline.ca, as part of the provincial direction for Ontario CCACs.
  • Community Connection’s Resource Specialists continue to work with CCAC to meet their provincial requirements for a bilingual online database.

 North Simcoe Muskoka Home and Community Care´╗┐ (310-2222)

  • Since 2010 Community Connection has been contracted by the North Simcoe Muskoka CCAC to provide information and referral requests received through their general office telephone and toll free phone lines. 
  • During 2010-2013 the CCAC marketed 211 as their public access point for community information and referral.
  • In 2013 the 310-2222 provincial phone number was launched for the public to contact the CCAC in their community, and the North Simcoe Muskoka CCAC began advertising 310-2222.
  • Community Connection’s Community Navigators continue to work with CCAC to meet their provincial requirements for bilingual information and referral.

 Ontario Works Search Portal – County of Simcoe

  • Community Connection’s Resource Specialists developed and maintain a custom, private search directory to support Ontario Works caseworkers, a program which provides financial and employment assistance to people in financial need.

Provincial Community Resources – Employment Ontario; Victim Support Services; Citizenship & Immigration Canada & Ontario

  • Community Connection’s Resource Specialists maintain community resources accessed by three provincial government services. The initiatives are led by Findhelp/211 for all of Ontario.
